The USHE form cannot be saved in progress so you must complete the entire form in one browser session or the information will be lost. Please allow 30 minutes to complete this form in one sitting.

After clicking on the link at the bottom of this assignment, expect the following:
- View the 9-min. Video: The Concurrent Enrollment Participation Overview Video.
- Complete the USHE Enrollment Form.
- You must receive an email confirmation of enrollment from the Utah System of Higher Education (USHE) before you can be assigned your University ID number and complete enrollment and registration for the Bridge Course.

To complete this form, you must have:

if you have one, a social security number, student VISA number, or alien registration number (your citizenship status does not affect your ability to participate in the CE program). For help finding your number, click here.
a parent or guardian present, to read disclosures about CE and grant approval for you to participate
access to both student and parent/guardian email accounts (both will receive an email with a verification code that is needed to complete the application)

Be sure to register for the correct university: THE UNIVERSITY OF UTAH. There is no USHE application fee to register for the University of Utah (if you see a payment screen for this step, you have selected the wrong university).
